Limitations

  • Ortho reports a bias with the following:
    • Desferal can cause a positive bias of 50%-80%
    • Ferrous sulphate can cause a negatie bias of 8%-18%
  • Ortho reports no significant effect with the following:
    • Bilirubin up to 27 g/dL
    • Hemoglobin up to 1000 mg/dL
    • Lipemia up to 800 mg/dL
  • The following interpretive data is visible to practitioner: Patients on Desferal show significant positive interference, samples containing ferrous sulfate give results with a negative bias.

Useful For

The measurements of serum iron and serum iron-binding capacity are useful in the differential diagnosis of anemia, iron deficiency anemia, thalassemia, possible sideroblastic anemia, and iron poisoning.  Total iron-binding capacity (TIBC) in serum, representing the iron-binding capacity of transferrin, is a useful index of nutritional status.  Serum TIBC varies in disorders of iron metabolism.  Serum TIBC is increased in iron deficiency and decreased in chronic inflammatory disorders, malignancies and hemochromatosis.
